MOBILE CRISIS CASE MANAGER

Summary of Duties

· Facilitate safety planning and support services

· Provide mobile response and crisis services to adults, children and families of Labette County

· Complete progress notes in accordance with CCBHC guidelines which meet requirements for medical necessity, goal-directed treatment, and patient response to treatment within prescribed timelines.

Labette Center will be looking for a person that is highly motivated and prepared for a new and innovative way to provide care.

We are seeking an enthusiastic, energetic, and motivated person to join our dynamic team as part of a Certified Community Behavioral Health Center (CCBHC). CCBHC’s provide equitable, high quality integrated behavioral health care that is sustainable. The CCBHC model is an outpatient model that provides integrated services including 24/7 crisis response and medication-assisted treatment. It also meets strict criteria regarding access, quality reporting, staffing, and coordination with social services, justice, and education systems.


SUD PEER MENTOR

We are seeking to add a Substance Use Disorder (SUD) Peer Specialist to our staff.  All applicants must be 18 years of age or older, have personal life experience receiving substance use support services, have a valid driver’s license, and be able to pass a background check.
